Overview
Retatrutide is an investigational Eli Lilly peptide that simultaneously activates GIP, GLP-1 and glucagon receptors. Not yet approved by any regulator.
Mechanism of action
Adds glucagon-receptor agonism to the incretin backbone, increasing energy expenditure and lipolysis in addition to appetite suppression and insulin sensitisation.
Evidence base
Phase 2 (NEJM, 2023) reported 24.2% mean body-weight reduction at 48 weeks on 12 mg weekly. Phase 3 TRIUMPH programme ongoing.

Safety & contraindications
GI adverse effects similar to other incretin therapies. Transient heart-rate increase observed at higher doses; long-term safety unestablished.
