Overview
SS-31, developed clinically as elamipretide (Bendavia), is a synthetic tetrapeptide that concentrates in the inner mitochondrial membrane by binding cardiolipin.
Mechanism of action
Stabilises cristae architecture, restores efficient electron transport, reduces reactive oxygen species and improves ATP production without acting as a free-radical scavenger itself.
Evidence base
Trials in primary mitochondrial myopathy, Barth syndrome and dry AMD have shown mixed but encouraging signals. FDA has considered accelerated approval pathways for rare mitochondrial indications.
