Overview
Semax is a heptapeptide derived from adrenocorticotropic hormone (ACTH 4–10), developed in the 1980s at the Russian Academy of Sciences and included on Russia's list of vital medicines.
Mechanism of action
Rapidly upregulates BDNF and NGF expression, modulates the melanocortin system without triggering cortisol release, and enhances dopaminergic and serotonergic tone.
Evidence base
Russian trials in ischaemic stroke show improved neurological recovery when administered within the first hours. ADHD, optic-nerve neuropathy and cognitive-decline studies exist but predominantly single-centre.
Typical dosing

Safety & contraindications
Generally well tolerated; occasional mild headache. Outside Russia used off-label with no regulatory approval.
